Items needed for registration:

Original state certified birth certificate

Social security card

Immunization on form 3231

3pt. screening on form 3300

proof of residence in the form of water, electric or gas bill

copy of sales contract can be taken temporarily from those just moving in or closing statement.

Utility bill will need to be provided asap.
