Columbia
County Board of Education provides $10,000 of coverage at
no cost to all eligible employees (employees who work 15 hours or
more a week). You may choose additional term life and permanent life
insurance for you, your spouse and your children. Our term life
coverage
is through
Lafayette Life Insurance Company. It includes
supplemental, spouse, and
dependent life plans. Our permanent life coverage is
through Transamerica. Payments for the amounts you elect
are made through easy payroll deductions. You are eligible
for this coverage if you work for your employer at least 30
hours per week and are under the age of 70. Please see below
explanations for each plan.
EMPLOYEE SUPPLEMENTAL
LIFE
You may choose
additional coverage up to five times your annual salary (rounded
to the next ten
thousand). The maximum coverage is $500,000 in coverage.
Coverage over three times the annual
salary will require a health questionnaire. The coverage
now includes portability, waiver of premium for disability and
an accelerated benefits option. Click

column is the amount of coverage
you have. Choice of $10,000 to $500,000,
not to exceed five times annual Base Salary.
Employee life insurance is
convertible/portable.
SPOUSE
LIFE
Spouse
coverage is based on the employee’s age and $10,000 - $100,000
amounts of coverage
can be purchased in $10,000 increments, not to exceed 50% of the
Employee benefit. These amounts are subject to

DEPENDENT
LIFE
Children over
14 days but less than 19 years, student dependent age, 19, but
less than 25 can be
covered with $10,000 worth of coverage. The cost for all
of your children that qualify is $1.28 monthly.
In order to obtain child coverage either the employee or spouse
must enroll for Supplemental Life coverage.
The Schedule of Benefits:
Accelerated Death Benefit
If you become terminally ill prior to age 64, you may elect
to receive 50% of your Life benefit under the contract.
Contract specifications apply. Your Life Insurance will
then be reduced by the amount of the benefit paid.
Guarantee
Issue
Evidence of Insurability will be required for all Supplemental
amounts:

All employee over age 60 will be limited to 50% of
the Guarantee Issue amount limits above. |
Evidence of Insurability Requirements
For amounts of up
to $125,000 over guarantee issue an Eligibility of Insurance
form is required. An Attending Physician Statement is
required only if there are questions. For amounts in
excess of $125,000 over guarantee issue, an Eligibility of
Insurance form, Attending Physician's Statement (if questions),
a Blood Test, and complete medical history are required
(employee cost).
Reductions
Employee: 35% @ 70, 55% @ 75, terminates at retirement.
Spouse: 35% @ 65, terminates the earlier of age 70 or when
employee ceases to be eligible.
Child(ren): Terminates at earlier of age 19 (25 for
full-time student) or when employee coverage terminates.
Conversion/Portability
If you leave your job, you can convert to a permanent policy of
individual insurance, or continue group coverage at current
rates.
Disability
If you become disabled prior to age 60, Lafayette Life will
waive the premiums as long as you remain disabled, but not
beyond age 65 as per contract specifications.
